'GET CRAFTY: FELT ART POUCH BAG' BY ANNABELLETTN (19 October 2025)

1 — Indemnity

I, the Participant and/ or parent/guardian of the participant hereby understand and certify that my/ my child's participation in 'Fashion | Art Workshops' and its activities are completely voluntary. 

I understand that although the Organizers and Instructors: AnnabelleTTN (hereinafter including and not limited to the organization, management, all staff, and other related personnel) have taken safety measures to minimize the risk of injury to workshop participants, none of Instructors can insure nor guarantee that the participants, equipments, premises, and/or activities to be free of hazards, accidents, and/or injuries. 

I acknowledge and accept that the aforementioned organizations will not be held liable for any injury, including death, that may be suffered by myself or my child/ward, which may arise either directly or indirectly from, or in connection with, 'Fashion | Art Workshops'. 

I hereby agree to indemnify the aforementioned organizations against any and all claims, charges, proceedings (whether civil or criminal), and liabilities arising from, or in connection with any injury that may be suffered by myself or my child/ward, or that my child/ward may cause to another person, whether arising under statute or common laws of Singapore or otherwise.

I also agree to indemnify the aforementioned organizations against any loss or damage to property, equipment or personal effects belonging to myself, my child/ward, or any other person and third party arising, either directly or indirectly from, or in connection with 'Fashion | Art Workshops'.

I hereby authorize organizers, their agents, and representatives to seek emergency medical attention on behalf of myself, or my child/ward in the event of an accident or medical emergency. While I understand organizers will attempt to contact me or my designated emergency contact, I agree the organizers shall be in no way responsible for the outcome of such medical treatment and hereby release them of all liability.

2 — Media Consent 

I, the Participant and/ or parent/guardian of the Participant understand that general photo-taking will commence throughout the event from Organizers and through Other Participants and Guests; Usage of general photographs and videos, for purposes such as publicizing the organization and the participants’ accomplishments to internal and external audiences, including in print and online, will naturally be permitted by default that the event is classified within public perimeters.

3 — Mailing Consent 

I, the Participant and/ or parent/guardian of the Participant, understand that by submitting this form, I will be automatically subscribed to the AnnabelleTTN mailing list. I understand that this mailing list will be used for the purpose of sharing updates on future workshops, events, and related activities. I may choose to unsubscribe at any time via the provided link in each communication.
